Abstract

Urticaria is suffering from 15 to 25% of the total human population. In the etiology of urticaria involves several allergens. In the pathogenesis of urticaria participate true allergic reactions and pseudoallergic reactions. For clinical assessment of activity of urticaria use UAS7 – an Activity Index of Urticaria. Not recommended for intensive survey of all patients with urticaria. National conciliation document recommends a wide range of diagnostic tests in chronic urticaria. Identify the main goals of treatment of urticaria: relief of symptoms of acute and chronic forms, the selection of adequate therapy of chronic urticaria. The H1-receptor antagonists are the only legitimate group of drugs for the treatment of patients with urticaria.

Abstract

Purpose: to study the changes in psychological status, autonomic nervous system, as well as their relationship to the level of biogenic amines blood arrhythmias of various types and origins on the background of a combined treatment of anti-arrhythmic and psychotropic drugs.

Materials and methods: surveyed, 95 patients (mean age of 45,2 +1,24 years), 42 men and 53 women with ekstrasystolichniy fibrillation, paroxysmal supraventricular tachycardia, atrial fibrillation on the background of coronary artery disease and vegetative dysfunction. Psychological testing conducted scales anxiety of Spilberger, depression Tsung, Hamilton, test RESINS, the content of biogenic amines in the serum were determined using a sensitive method in the sample.

Results: the revealed disorders of autonomic balance, increased levels of anxiety, depression, poorer quality of life, reducing the protective functions of the CRL of the body.

Summary: combined therapy, including activators SLS had a more pronounced antiarrhythmic effect, contributed to the stabilization of the ANS, and resolved the levels of biogenic amines is more effective than on the background of basic therapy.

Abstract

Purpose: studying of endotoxin antibodies level of classes G, M, A and level of C-reactive protein at patients on a programhemodialysis and a constant peritoneal dialysis.

Materials and methods:16 patients with a chronic kidneys disease 5 stage on a peritoneal dialysis and 62 patients on a program hemodialysis are surveyed. Level endotoxin antibody classes G, M, A, and C-reactive protein was determined by ELISA.

Results: it is established that patients on a peritonial dialysis have level anti-ET-IgG in 4.9 times in comparison with group of conditionally healthy patients, and levels anti-ET-IgA and anti-ET-IgM didn’t differ from norm indicators. Also increase of level of C-reactive protein in 8.8 times, in comparison with indicators of healthy donors. At patients on a program hemodialysis increase in 5.6 times anti-ET-IgG, and in 8 times of C-reactive protein in comparison with group of healthy patients. And levels anti-ET-IgA and anti-ET-IgM also didn’t differ from norm indicators. Levels of endotoxin antibodies and C-reactive protein at patients on a hemodialysis and on a peritonial dialysis among didn’t differ.

Conclusion: in both groups of patients found an increased antibody response to ET (on a high level of anti-ET-IgG) against the background of a high level of systemic inflammation (at the level of C-reactive protein). There is chronic endotoxin aggression in hemodialysis patients.

Abstract

Purpose: to evaluate the different methods of prevention of iron deficiency anemia in breast-fed infants.

Materials and methods: studied questionnaires of 948 lactating mothers, determined the hemoglobin level in their infants, the serum ferritin and serum iron level, the total iron binding capacity of serum (n=96), the breast milk iron concentration (n=88).

Results: the median of breast milk iron concentration was independent of iron supplementation in lactating mother’s (2.1 μmol/L vs. 3.4 μmol/L, р=0.16). Iron status was better in breast-fed infants with iron supplementation.

Summary: sron supplementation with of 1 mg/kg, starting at 4 months of age during 2 months in exclusively breast-fed infants is more effective compared to iron-containing vitamin and mineral complexes intake in lactating mothers.

Abstract

Purpose: identification of factors predisposing to the development of hypertrophy of pharyngeal tonsil at children at the age of 3-14 years.

Materials and methods: 774 children at the age of3-14 years are surveyed. The retrospective method of information collection is used, is carried out the somatometry of children, the time of mucotsilliarny transport is investigated.

Results: adenoids, acute and chronic adenoiditis are among the most common diseases of LOR organs in childhood. Taking into account the phenotypic characteristics identified three clusters of children at risk of pharyngeal tonsil hypertrophy. Patients third cluster pharyngeal tonsil hypertrophy II-III art. was detected in 80.5% of cases and unfavorable personal atopic anamnesis occurred in 87.3% of cases. The vast majority of children in this cluster accounted for sickly children (71.9%), the average VMTST - 28 ± 2,38min. In 56.5% of the children diagnosed MeMaS body type.

Summary: identified clusters of children with a hight risk of adenoids.

Abstract

Purpose: identifying patterns of changes in the general and local antioxidant index in the dynamics of experimental dysbiosis.

Materials and methods: the model of the experimental dysbiosis was reproduced on 200 male mice, by means of intraperitoneal injection of gentamicin The control group consisted of 50 intact animals. In coprofiltrates and in the blood of mice, there was examined the activity of antioxidant enzymes (superoxide dismutase and catalase) and content of malondialdehyde. The biochemical parameters were assessed photometrically at an optical density value of the samples. The miicrobial landscape of feces was determined by classical bacteriological method. All figures were examined on the 3, 10, 17, 24, 30 day after the 5-day course of antibiotics.

Results: it was found that by an increase in the total number of opportunistic microbes as part of the large intestine microbiocenosis, a reduction in general and local antioxidant index takes place, with a significant advantage of the latter.

Conclusion: on the basis of the calculation of the local antioxidant index there was proposed screening criterion to evaluate the state of the colonization resistance of the large intestine with the establishment of the risk of chronic systemic inflammation development.

Abstract

Purpose: to study in patients with chronic hepatitis C the pathogenesis hematopoietic activities of autologous blood hemolysate.

Materials and methods: we made an indirect MALD-TOF-mass spectrometry of blood plasma in 37 patients with chronic hepatitis C who received the autologous blood hemolysate. The study was performed twice: before autohemotherapy and after 10 days of daily intravenous injections of 20.0 ml of the autologous blood hemolysate.

Results: succeeded in isolating a blood plasma over 300 polypeptide sequence. Thus, as a result of intravenous administration of 20.0 ml autologous blood hemolysate frequency of IL-1β, IL-3, IL-6, IL-9, IL-11 and erythroferron significantly increased and the IL-10 and IL-13, by contrast, dropped .

Summary: identification of proteomic changes in the profile of blood plasma in the predominance of proteins that stimulate hematopoiesis probably accounts for the clinical effectiveness of the autologous blood hemolysate.

Abstract

Purpose: the personification approach to the choice of antisecretory drugs in chronic gastroduodenitis, gastroesophagealreflux disease in children.

Materials and methods: The study involved 70 children (12-14 years) with chronic inflammatory diseases of the upper digestive tract (WOCAT) (gastroesophageal reflux disease (GERD) in conjunction with chronic gastroduodenitis (CGD)). All children performed esophagogastroduodenoscopy (machine Olympus) with biopsy mucosa (CO) of the distal esophagus, gastric antrum, duodenum and molecular genetic research aimed at identifying gene polymorphism CYP2C19 * 2.

Results: 50.2% of children surveyed were identified, rapid metabolizers (RM), while 28.4% of intermediate metabolizers (PM) of proton pump inhibitors (PPIs), while slow metabolizers (SM) IPP in 21.4% of cases .

Conclusion: the continuing high activity of gastritis after standard therapy in children with RM/ PM IPP suggests the need for a personalized approach to the choice of antisecretory therapy based on gene polymorphism CYP2C19*2.

Abstract

Purpose: testing the use of hyperbaric rerespiration (HRR) to optimize respiratory function (RF) patients with chronic diseases of the lower respiratory tract (CDLRF).

Materials and methods: surveyed 52 patients (29 men and 23 women) with CDLRF at the age of 23-55 years. After knocking over the aggravation of the disease (surgery), patients basic group (BG, 38 persons) were made HRR-course: 12-14 procedures of 45-50-min stay under pressure (152 kPa) when breathing through an additional ≪dead≫ space that shaped the terms of hyperoxia and hypercapnia. In the control group (CG, 14 people) held the standard rehabilitation.

Results: application of HRR in patients BG was accompanied by significantly greater (p < 0.05) increased volumetric and highspeed RF indicators than in CG. The effects of treatment in patients OG were significantly more resistant and stable.

Summary: HRR is an effective and safe method to optimize RF patients with various forms of CDLRF.

Abstract

Purpose: to study features of ultrasound parameters of the joints and vascular endothelial growth factor (VEGF) in patients with rheumatoid arthritis, depending of the level of antibodies to cyclic citrullinated peptide (anti-CCP) in the blood.

Materials and Methods: 155 patients with rheumatoid arthritis joints had done ultrasound with Doppler of the joints, the levels of anti-CCP and VEGF in the blood - ELISA. The patients were divided into two group: 1st gr. - patients with negative or low positive level of anti-CCP ( ≤ 40 IU/ml) in the blood, 2nd gr. patients with highly positive level of anti-CCP ( > 40 IU/ml).

Results: In patients with anti-CCP levels > 40 IU/ml were to increase the level of VEGF in 1.5 times, higher effusion of joints, angiogenesis (vascularization of synovium in 2 times was more likely gradation 2-3 points), were more tendosynovitis and erosions compared with patients with low levels of anti-CCP.

Conclusion: the concentration of VEGF in the blood, the intensity of synovial angiogenesis, effusion in the cavity of joint, tendosynovitis and erosion were to increase with had been increasing levels of anti-CCP in the blood.

Abstract

Purpose: the assessment of place of the endothelial vasoregulation factors (nitrogen oxide (no) and endothelin 1 (et-1)) in the formation of the arterial hypertension (AH) in condition of chronic stress (CS) was held.

Materials and methods: 160 railroad engineers (RRE) were examined. They had work experience from 1 to 35 years. As controls, 100 healthy volunteers wereexamined. The level of adrenocorticotropic hormone, cortisol and corticosterone in the blood as a condition of the stress system (SS) were measured.

Results: it was found out that the effect of stress reaction is accompanied by the serial reaction of the SS: activation period, adaptation and disadaptation. In the activation and the adaptation periods was marked increase synthesis of NO against the background moderate increase of ET-1 concentration. The disadaptation period was accompanied by decrease in NO production and doubling the level of ET-1, an increase of the thickness of the carotid intima-media complex (as evidence of vascular remodeling processes). Insufficient synthesis of the vasodilators, excessive production of the vasoconstrictors, remodeling of the vascular wall contributed to the formation, process and progression of the hypertension.

Conclusion: these results specify the pathogenic mechanisms of AH in condition of CS.

Abstract

Purpose: to improve the results of surgical treatment in patients with epithelized pilonidal sinus (EPS) by developing and incalcating an effective surgical method comprising measures for preventing recurrence.

Materials and methods: 60 patients with EPS and sacrococcygeal fistulas have been operated, of the total number of 60,in 22 patients EPS excision was followed by suturing the wound edges to the bottom; in 10 patients the wounds were left open, in 9 excision supplemented bywound defect plastics with displaced non-free skin flaps in the developed modifications.The choice of operation method depended on the extent of the inflammatory process, duration of the disease, a history of recurrence.

Results: 3 cases of recurrence (30%) were registered in the I group of patients; 7 cases of recurrence(31,8%) – in the II group; 4 cases of recurrence (44., %) – in the III group; 3 cases of recurrence (15,8 %) - in the IV group.

Conclusion: radical surgery with simultaneous closure of the surgical wound with non-free skin flapsare the most promising for improving long-term results in EPS treatment (including recurrent fistulas), as evidenced by the immediate and long-term results.

Abstract

Purpose: study of correlative relation between homologous diastolic components of spectogram of regular impulsive-wave and fabric Doppler, developement of normative values of new diastolic indicators el and al considering age and gender, also comparison of values of new diastolic indicators el and al persons and patients with characteristics of diastolic dysfunction

Materials and Methods: transthoracic Doppler’s echocardiography was tried on 156 patients at age 21-88 afflicted with different cardiac pathology -91. In the article results are presented in the article that demonstrate occurrence of strong relation between diastolic high-speed indicators of movement of fibrous ring mitral valve’s (el and el’), registered with spectrogram of regular impulsive-wave and fabric Doppler and normative values are also set of indicators el and al, considering age and genger.

Results: with development of left ventricle’s diastolic dysfunction the value of el authentically reduces considering gender.

Conclusion: impulsive-wave Doppler can be used for diastolic function of the left ventricle of heart assessment in cases when fabric Doppler in inaccessible.

Abstract

Purpose: to study relation between middle rectal and uterine arteries in norm and with uterine fibroids.

Materials and methods: the analysis of angiograms of 63 patients with the uterine fibroids and without pathology (n=29 and n=34 respectively) was performed. Localization of orifices of middle rectal arteries, their diameter, existence anastomosis with uterine arteries are estimated.

Results: the right middle rectal artery generally departed at level or earlier than uterine artery (26,3% and 21,1% respectively), and left was more often above the orifice of a uterine artery (42,1%). Diameter of rectal vessels didn’t differ in both groups (р>0,05). Anastomosis between middle rectal arteries and uterine vessels were better developed in the I group (52,4% of  upervision).

Conclusion: the network of anastomosis between middle rectal arteries and uterine arteries was more developed in I group (patients with uterus myoma), that caused higher risk of inappropriate embolism. Knowing features of orifices relation helps to reduce time of endovascular intervention and lower radiation influence.

Abstract

Purpose: assess the benefits of a differentiated approach in the treatment of chronic endometritis.

Materials and methods: hysteroscopic visualization of macrotype HAE (hyperplastic, hypoplastic, and mixed) specific to each pathologic basis allows you to optimize the effectiveness diagnostic search of the disease.

Results: comparison of immunoreactivity with HAE with variants of adaptive reactions showed characteristic for each macrotype the failure of the immune response.

Conclusion: the dependence of variability in the response of the female body from different directional effects of rehabilitation therapy. Proven efficiency pathogenetic therapy in the amelioration of immunodeficiency or autoimmune reactions contributing to the chronicity of the inflammatory process in the endometrium.

Abstract

Purpose: to investigate of mortality from respiratory diseases of children aged 0-19 at Rostov region during 2004-2013.

Materials and methods: official statistics annual reports of Rostovstat were analyzed.

Results: the mortality from respiratory diseases took 8th place in 2013 among all causes of mortality (25 from 779 dead; 3,2% or 2,983 per 100 000 population aged 0-19).

Conclusion: children 1st year of life accounted for the majority of them – 64%. The leading cause of mortality was pneumonia; pneumonia without of specifying the pathogen accounted for majority of them. It was found dynamic of decreasing of mortality from respiratory diseases on 72,3% from 11,636 (48 children) till 3,224 (16) per 10 000 live birth. The leading cause was pneumonia without of specifying the pathogen. Decreasing of mortality was from 10,182 per 10 000 live birth (42 dead) till 2,821 (14).

Abstract

Purppose: the effect of the complex of therapeutic exercises on the multiplicity ARD, cellular and humoral immunity in patients with infantile cerebral paralysis (ICP).

Materials and methods: we studied the immune status of 70 patients with cerebral palsy with spastic diplegia. In the main group (35 children) for a comprehensive treatment were added classes on copyright simulators, in the comparison group (35 children) was not used copyright simulators.

Results: application author’s techniques could substantially reduce the incidence of acute respiratory infections.

Conclusion: the proven positive impact of physiotherapy on humoral and cellular immunity, reducing the ratio of OCR in the main group.

Abstract

Purpose: development of the ways to optimize the treatment tactics for parotideomassetericphlegmons.

Materials and methods: anatomic studies were carried out on 45 corpses following an original method (Russian Patent No.2271740), and 86 patients were examined. The patients were divided into two groups: control (42 patients) and main (42 patients), the latter treated in accordance with the developed method.

Results: in cases of parotideomassetericphlegmon, elevation of the interstitial pressure (IP) by 20-30 mm Hg above the normal is an indication for fasciotomy. The second stage of the operation was reduced to 5 days for the main group (vs 7 days in the controls). Distant good results were obtained in 90.9% of the patients (54.8% in the controls).

Abstract

Among all forms of intestinal obstruction, colonic obstruction make 30-40 %. 93% of colonic obstruction is tumorous colonic obstruction. In this report we overview results of diagnostics and treatment of 226 patients with different rate of tumorous colonic obstruction. Results: in case of effectiveness of nonsurgical treatment, it would be well to resect a primary tumor; in case of ineffectiveness of nonsurgical treatment - would be well to form a stoma; resectable metastases in liver or lungs don’t hamper the resect a primary tumor.

Abstract

Purpose: to characterize role interaction ≪the medical worker – patient≫ in system of medical care to the people living with HIV to estimate their influence on value.

Materials and methods: in 2012-2014 voluntary anonymous individual internal questioning of 187 HIV-positive people of the patient and 92 health workers from various treatment-and-prophylactic establishments of Volgograd and Volgograd region is carried out.

Results: the characteristic of relationship of health workers and HIV is given – the infected patients arising in the course of performance of the professional roles by them their influence on value judgment of quality of medical care is established.

Conclusion: monitoring of satisfaction of HIV – the infected patients of various treatment-and- prophylactic establishment by quality of medical care provided to them, development of the establishments which achieved the best indicators is required.

Abstract

The paper presents results of research on the analysis of the structure and performance of hospital beds hospital Rostov State Medical University. Identified unexpressed systematic approach to the reorganization of hospital beds. The method of calculation of the possible reduction and conversion of the number of beds in the studied institutions. The calculation was based on the use of actual, planned and standard indicators of hospital performance.
