Abstract

Statement of hormonal activity and potential malignancy are the most important issues of diagnosis and selection of appropriate treatment way for patients with adrenal tumors. Adrenal cortical carcinoma (ACC) is rare disease with poor prognosis. Incidience of ACC is about 0.5–2per million, and no more then 2–4% among adrenal tumors. Th is draft of guidelines of diagnosis and treatment of ACC is off ered to common discussion among wide round of specialists.

Abstract

Th e article off ers an analysis of the literature of recent years dedicated to the development and prevalence of Helicobacter pylori resistance to various antibiotics, which are used for the eradication of this microorganism. Th e reasons of the growth of Helicobacter pylori antibiotic resistance at the present stage have been considered. Th e author describes specifi c issues surrounding diff erent therapeutic options, the ways to overcome this pathogen’s resistance, and the necessary steps in choosing the correct eradication regimens, according to the level of Helicobacter pylori antibiotic resistance. Particular attention is paid to the prevalence of antibiotic resistance of Helicobacter pylori in childhood.

Abstract

Purpose: to identify the trigger group nosology in newborns, acting on which it is possible to reduce infant mortality and to assess the links between the development of complications and the period from birth to transfer to the surgical ward in congenital intestinal obstruction due to atresia of the intestinal tube.

Materials and methods: the structure of the post-mortem diagnosis of infants with surgical diseases of the fi veear period, as well as medical cards of 95 infants with atresia of the intestinal tube with a favorable outcome of treatment.

Results: one of the trigger groups are congenital malformations, accompanied by obstruction of the digestive tract; a statistically signifi cantly higher incidence of complications such as postoperative pneumonia in infants with atresia of the small intestine and rectum high atresia transferred to the surgical ward aft er 24 hours of birth.

Summary: to reduce the infant mortality rate is necessary, including, establish regulatory requirements for a transfer to a children’s hospital surgical neonates with atresia of the small intestine and colon atresia highest within the fi rst 24 hours of birth.

Abstract

Purpose: to evaluate the daily urinary excretion of markers of infl ammation (IL-1β, MCP-1) and transforming growth factor (TGF-β1) in women with type 1 diabetes with diff erent levels of daily albuminuria in diff erent trimesters of pregnancy.

Materials and methods: the study involved 48 women with type 1 diabetes and 24 women with physiological pregnancy in diff erent trimesters of pregnancy. Quantitative determination of the daily urinary excretion of IL-1β, MCP-1 and TGF-β1 was measured by enzyme-linked immunosorbent assay (ELISA).

Results: pregnant women with type 1 diabetes with normoalbuminuria (NAU) have comparable values of urinary excretion of IL-1β, MCP-1 and TGF-β1 with healthy pregnant women in all trimesters of pregnancy (r>0,05). In groups of pregnant women with microalbuminuria (MAU) and proteinuria (PU) there are an intensive increase in excretion of IL-1β, MCP-1 and TGF-β1.

Summary: level of IL-1β, MCP-1 and TGF-β1 in pregnant women with type 1 diabetes with MAU and PU signifi cantly higher than in the group with NAU in all trimesters, in the group with NAU they are comparable with the results of healthy pregnant women.

Abstract

Purpose: to evaluate the eff ectiveness of diff erent chemotherapy variants of NHL patients.

Materials and methods: the therapy of 64 B-cell NHL patients aged 21-83 (38 males and 28 females) has been analysed. Th e eff ectiveness of the treatment was evaluated aft er the third cycle of therapy and inductional course of treatment as well as aft er the completion of the entire treatment program.

Results: general eff ectiveness of the treatment made 80% (20 patients), full remission made 52% (15 patients), partial remission made 28% (7 patients), stabilization was achieved in 2 cases (2 patients) (8%), in three cases (3 patients) there was progressiveness of the disease. In the structure of aft er-eff ects the prevailing ones were: hematologic toxicity III-IVst. (56% of the patients), infection (48% of the patients), hemorrhagic syndrome (13% of the patients).

Summary: contemporary PCT’s conducted according to prognostic risk groups, allows to achieve high survival rates for the vast majority of patients with NHL.

Abstract

Purpose: to develop a science-based methodology for determining the need for infusion solutions needed to provide emergency medical care and follow-up treatment of the aff ected population in the liquidation of the health consequences of emergencies.

Materials and methods: the analysis of legislative and regulatory framework in а sphere of public drug coverage for emergency medical assistance in emergencies. Th e methodological basis of research were the principles of the system, an integrated, process, logical framework approach to study the organization of activities of pharmacy chains in emergencies.

Results: putting into practice the proposed evidence-based management allows health authorities and State Disaster Medicine Service plan and calculate an amount need of infusion solutions for emergency medical care in the aft ermath of the loss of health in emergencies. Application of the proposed method to optimize rational use of budgetary funds for the establishment of local, district and regional reserves infusion solutions for the liquidation of consequences of emergency situations.

Summary: the proposed planning approach of minimum reserves of intravenous fl uids to provide intensive therapy requires further improvement of theoretical and methodological basis.

Abstract

Purpose: to study a condition of the immune status at patients with a cutaneous squamous cell carcinoma in the postoperative period.

Materials and methods: changes of the immune status it is carried out at 39 patients with a squamous cell carcinoma of the skin. Th e age of patients made 65 Ѓ} 3 years. At all patients the stage of a disease made T1-2N0M0. Researches were conducted before expeditious treatment, in the fi rst postoperative days, for the fi ft h, seventh, tenth postoperative days and in 18 months aft er the undergone operation. When studying the immune status the main indicators humoral and cellular links of immunity were defi ned.

Results: in a cellular link of system of immunity at patients with a squamous cell carcinoma of the skin before the carriedout expeditious treatment decrease in both relative, and absolute number of the lymphocytes bearing receptors of CD16+, CD3+,CD4+, CD8+ which considerably increased by the tenth postoperative days is noted. In a humoral link of system of immunity at patients with a squamous cell carcinoma of the skin decrease in quantity of IgG, M, and also total of a complement, its S3-and C4 fractions, a tumor necrosis factor, SILT 1β, 6,10, interferon, a phagocytic index is registered, a Nitroblue tetrazolium spontaneous and stimulated by Zymosanum, thus the increase in the circulating immune complexes was noted.

Summary: partial restoration of IgG, M happens for the tenth postoperative days.

Abstract

Purpose: to evaluate the diagnostic value of plasma homocysteine and lipoprotein cholesterol fractions in dynamics of neurological disorders in the(не нужно) early recovery period of ischemic stroke.

Materials and Methods: 70 patients with ischemic stroke prescription of 1-4 months and 20 control subjects with atherosclerotic encephalopathy I st were examined. Methods of observation of patients included some clinical, neurological, neuroimaging, laboratory and statistical tests.

Results: the autonomic dysfunction and focal neurological defi citwere predominated in a structure of the revealed neurological symptoms. An increase of blood levels of total cholesterol, LDL, VLDL and IA, GC were registered in all patients in comparison with control group (p <0,001).

Summary: the study found out that in the early recovery period of ischemic stroke is determined by the increase in blood levels of total cholesterol, low-density lipoprotein cholesterol and very low density lipoproteins, triglycerides, atherogenic index, which is combined with an increase in plasma homocysteine. A direct correlation between the index of atherogenic blood and plasma homocysteine levels; homocysteine blood levels and low density lipoprotein cholesterol in patients which has a certain diagnostic and possibly prognostic value.

Purpose: Th e study of the West Nile Fever (WNF) with the estimation of results of ecological/epizootological monitoring and epidemic manifestations of infection.

Materials and methods: investigations for the presence of antigen of WN virus were carried out on 5754 specimen (4187 samples of brain suspensions) of birds belonging to 90 species, 15 orders; 4153 specimen (1038 samples of brain suspensions) of 17 species of mammals; 46113 specimen (2081) of 18 species of mosquitoes; 13883 specimen (1588) of imago ticks of six species, and others (2001-2013). Methods used: enzyme-linked immunosorbent assay, epidemiological analysis.

Results: the territories of risk for WNF were defi ned with regard to bird ecology, fauna of mosquitoes of Culicidae family was characterized, the ecological linkages of WN virus with birds, mosquitoes and mammals were defi ned. Th e area of habitation of West Nile (WN) virus was determined. Epizootological/epidemiological zoning of the Rostov Region was carried out with identifi cation of territories, diff ering by the degree of risk of population infection with WN virus.

Summary: Th e results obtained can serve the evidence of formation of natural and anthropurgic foci of WNF.

Purpose: the analysis of carbohydrate-energetic metabolism of vaginal tissue taken in conjunction with estimation of interrelations between fatty acids composition and effi cacy of Mg2+and Ca2+-dependent ATP-ase working at women with atrophic colpitis in postreproductive period.

Materials and methods: it was established the vaginal tissue of 35 women (age of 51 Ѓ} 2,8лет) with signs of light atrophia. Th e comparative group – 35 women (age of 48 Ѓ} 3,6лет) without pathologic urogenital symptoms. It was established the activity of enzymes of carbohydrate and energetic metabolism, the activity of Ca2+, Mg2+-dependent ATP-ase and composition of fatty acids of vaginal tissue.

Results: iIt was found the changes of enzymes of carbohydrate and energetic metabolism at atrophic colpitis. In this situation it was found the increasing of polyunsaturated fatty acids. Th is changes combines with increasing activity of Ca2+, Mg2+-dependent ATP-ase.

Summary: results of investigation suggest formation of pathogenic chain in atrophic colpitis. Th is chain binds with change of priority direction of diff erent metabolic pathways, aff ection of cell transport systems work that lead to diff erent system changes on cell level which may be connect not only with aff ection of steroidogenesis but also with disturbance in hormone-receptor sending of diff erent signal systems.

Abstract

Purpose: to evaluate the eff ectiveness of treatment of facial pain based on the study of the mechanism of neurogenic pain symptoms in rhinosinusitis.

Materials and methods: the study includes 380 patients with rhinosinusitis with varying severity of pain symptoms . Age subjects ranged from 15 to 81 years. Th e investigation of the level of substance P in plasma.

Results: in excess of the high end - tration of substance P in the serum of patients with rhinosinusitis more than 2000 pg / ml refl ect participation in the infl ammatory process of neurogenic mechanisms and require pathogenetic therapy.

Summary: treatment of neurogenic infl ammation was performed drugs prednisolone and Milgamma. Relief of neurogenic component of the infl ammatory process in rhinosinusitis improves the eff ectiveness of treatment and reduce the time to 3-5 days.

Abstract

Purpose: to determine the nature of the impact of injecting drug use on cellular immunity and assessment of the importance of drug use as a factor contributing to HIV infection and aff ect the course of disease in HIV- positive patients.

Materials and methods: surveyed 144 people - 109 patients with drug addiction, HIV infection and HIV - positive injecting drug users and 35 healthy individuals.

Results: found that injecting drug potentiates caused by HIV infection decrease the percentage of helper cells ( CD4 + lymphocytes ), natural killer T (CD3 +16 +56 + lymphocytes), natural killer cells (CD3- 16 +56 + cells ), increased cytotoxic T lymphocytes (CD8 + lymphocytes), the proportion and absolute number of T- lymphocyte activation characteristics (CD3 + HLA-DR + cells).

Summary: injecting drug use increases the risk of infection among injecting drug users (IDUs) without HIV, and decreases the antiviral response in HIV- positive patients, a statistically signifi cant potentiating caused by HIV in the blood decrease in helper cells, and natural killer T- natural, regulatory cells, immune hyperactivation that contributes to the progression of the disease.

Abstract

Purpose: to improve on the results of biliodigestive (BD) reconstructive operations for benign biliary strictures using of arefl ux modifi cation of hepatico-/choledochojejunostomy (HJS/CJS) (Russian Fed.Patent No2470592) .

Material and methods; a valve mechanism of proposed BD-anastomosis was studied on 14 organ complexes. Clinical researches based on comparable studies of postoperative period in two groupsof patients: I (control group) included 44 patients operated on using standard surgical procedure, II – 31 patients were undergone BD reconstruction with arefl ux valve.

Results: estimation of postoperative period by such parameters as dynamics of blood analyses, complications, symptoms of dyspepsia and quality of life showed advantages in II group of patients.

Summary: use of arefl ux modifi cation of HJS/CJS allows reach the better results of postoperative period on parameters of comlications and quality of life in compare with standard method of BD reconstruction.
