The article presents the findings of agent’s role macrophage migration inhibitory factor in pathogenesis of different pathology, as well as the significance in prediction of the coarse and outcome of the disease.
The article provides scientific data on pathogenetic mechanisms of atopic dermatitis formation with regard to the influence of microorganisms of the gastro-intestinal tract on the formation of immunity in infancy; the role of breastfeeding in the prevention of atopy is emphasized.
Peculiarities of visceral pain and its pathogenesis are analyzed in the article. The article presents role of receptor apparatus, which accepts algogenic information, conducting structures role of spinal cord and brain in the formation of visceral pain. The pain mediators, neurotransmitters of spinal cord, and the theory of referred pain are analyzed.
In the present literary revive data about pregnancy and childbirth in women late reproductive age (35-49 years). The article considers the questions of the psychological status, age-related changes of the female reproductive system and combined extragenital pathologies on the course of pregnancy and childbirth and on a further development of children in patients of this age group. The biological modeling of pathology of pregnancy and childbirth may be used to solve a number of fundamental and practical problems of modern obstetrics.

Purpose: To study post erectile dysfunction (ED) in the dynamics of the patients operated on various ways for a herniated disc of the lumbar-sacral region of spine.
Materials and methods: 34 male patients were operated. All the patients before and after surgery were evaluated the severity of pain, the degree of erectile dysfunction, neurological outcome. The patients were divided into 2 groups: microdiskectomy without curettage cavity disk with prevention of epidural fibrosis by fat with injections of Depo-Medroli - group I, microdiskectomy without curettage cavity disk group - II.
Results: Erectile dysfunction in patients with disc herniation of the lumbosacral region of spine in our study occurred in 57.7% of cases. In 38.2% of patients was revealed moderate and severe ED.
Summary: Results of surgical treatment in the first group of patients were some what better than those of the second. This is associated with a lower incidence in the first group of recurrence of disc herniation and scar-adhesions epiduritis in the postoperative period.
Purpose: the experiment on laboratory animals to study the efficacy of ultrasonic scalpel «Ultraсision Harmonic Scalpel» compared with monopolar electrocoagulation.
Material and methods: the study of morphofunctional changes in the liver, with the application of Ultraсision Scalpel «Ultraсision Harmonic Scalpel» and monopolar electrocautery in the experiment on 16 white rats weighing 200 to + 20 grams.
Results: ultrasound, compared with electrocoagulation, has a more gentle effect on organs that prevents necrosis tissue damage to the small blood vessels and extrahepatic bile duct.
Summary: in the experiment proved the effectiveness of ultrasonic scalpel (USS) operations on the liver and bile ways, proved intact of the latter in relation to the functions of the liver, which demonstrates its advantages over traditional monopolar coagulation.
Purpose: To characterise sonographic pattern depending on the duration of complicated chronic pancreatitis.
Materials and Methods: The ultrasound research with the machine Kransbuller Sonoscop 30 was conducted on 50 patients. In this cases, the size of the pancreas, its contours and character echostructure, parenchymal echogenicity, the diameter of Wirsung duct, the state of its walls, the presence of inclusions were investigated.
Results: In complicated forms of pancreatitis in the duration of 3 year old ultrasonic signs of inflammation of the pancreas are defined and in the duration of more than 3 year, the outlines of the pancrease was rough but clear, gland was deformed, more pronounced dilatation of the Wirsung duct observed, along with local expansion. there are large areas of dense tissue, and chronization process is accompanied by development of fibrosis.
Summary: Pseudocyst formation is observed in 81.8% of patients for 3 years duration of pancreatitis and in 42.9% with duration more than 3 years. The majority of pancreas ducts stones found in the patients whose duration of the disease was more than 3 years - 42 9% versus 18.2%.
Purpose: development of diagnostic criteria of the sepsis, allowing early appointment of selenase, at newborns with the respiratory distress syndrome (RDS) being on artificial ventilation of lungs (AVL).
Materials and methods: controlled randomized blind clinical test of application of selenase at newborns with RDS and perinatal defeat of TsNS being on the AVL was carried out. 100 newborn children being in a critical condition were surveyed.
Results: during the study, the method of «decision trees», formulated diagnostic rules of allocation of sepsis, with an accuracy of 77,78%, sensitivity of 84% and specificity of 75,38%.
Summary: The use of diagnostic rules will be suspected sepsis, at the stage of admission to the intensive care unit, which, in turn, serve as the basis for appointment selenazy and will assist in those patients who need selenium first.
Purpose: To reduce the economic costs of treatment of newborns in critical condition, using selenium, based on reducing the duration of carried out IVL and hospitalization.
Materials and methods: controlled randomized blinded clinical test of selenase infants with RDS and perinatal CNS, are on mechanical ventilation (mechanical ventilation) was carried out. 76 newborn children being in a critical condition were surveyed.
Randomization was carried out by a method of the computer program of a random number generator. In I group (n=36), except standard intensive therapy accepted in office (hospital), entered selenase in a dose of 10 mkg/kg/days intravenously slowly, within 10 days, II group (n=40) – group of control.
Results: during the test it was found and statistically proved that the group of children receiving selenase, duration of IVL and ongoing hospital stay was, respectively, 1.6 and 1.36 times less (p <0.05) than in the group of control.
Summary: the use of selenase at children with respiratory pathology helps to reduce the financial cost of treatment at 1.6 times and has pharmacoeconomic lucrative nature.
Purpose: Research of certain peculiarities of the clinical course of atopic dermatitis in children’s early age.
Materials and Methods: A total of 30 children in early age with the clinical manifestations of atopic dermatitis were involved in the investigation.
Results and Summary: It was found that сlinical manifestations of atopic dermatitis in early infancy is a predictor of severe disease.In the genesis of atopic authentic dermatitis endogenous factor is inherited predisposition.The violation of a hypoallergenic diet of nursing mothers is the main trigger and exacerbations predisposing factors of Atopic Dermatitis. The severity of the disease correlate with the gastrointestinal tract lesions, which explains a comprehensive treatment of this pathology.Due to frequent exacerbations of the disease, children with moderate to severe forms of atopic dermatitis are not carried out an adequate vaccination in accordance with the National Immunization Calendar.
Purpose: to explore results of chronically implanted endocardial pacemaker leads extraction with extracorporeal circulation and using sternotomy pump off approach.
Methods: bacteriological analysis and electronical microscopy of deimplanted pacing leads were estimated. The short-distance outcomes of surgical treatment are analyzed too.
Results: 26 patients were operated and 59 leads extracted. The vast majority of infectious agents were presented by Streptococcus viridians and Staphylococcus epidermidis. One patient had early postoperative bleeding. Another case was complicated by relapse of infection. There wasn’t mortality observed.
Conclusion: the pump off approach of extracorporeal circulation is a safe and effective method for endocardial pacemaker leads extraction.
Purpose: To study the pathogenetic significance of cellular factors in the development of innate immunity mediated by HIV infection
Materials and Methods: Oxygen-dependent metabolism of neutrophil granulocytes, the cytolytic activity of NK cells, production of IFN – γ were studied. We observed 36 HIV-infected patients.
Results and Summary: It is shown that after the stage of activation of cellular innate antiviral defense mechanisms formed functional suppression, making a significant contribution to the progression of HIV infection.
Purpose: To study dynamics of a lipidic profile at patients with diabetes 2 types, against combination application of exenatide and metformin.
Materials and methods: Research is conducted on 87 patients with diabetes 2 types complicated steatosis. All patients received general clinical research such as lipidogram.
Results: In the course of treatment patients had a decrease in atherogenous fractions of the cholesterol, more expressed at treatment metformin and combinations metformin with the exenatide. The combination metformin with preparations sulfonylurea concerning correction of indicators of a lipidic exchange was less effective.
Summary: more effective constrain hypertriglyceridemia as a risk factor for steatosis observed with metformin and exenatide.
Purpose: to study antioxidant enzymatic state of rythrocytes at arterial hypertension (AH) at teenagers.
Materials and methods: venous blood taken from cubital vein and stabilized by heparin was used for investigation. 20 teenagers from 13 to 16 years was examinated. Main antioxidant enzymes: superoxidedismutase (SOD), catalase, glutathione reductase (GR), glutathione peroxidase (GPO) and level of reductive form of glutathione (GSH) were investigated in the purpose to estimate the state of antioxidant system.
Results: teenagers with AH has had dysbalance in the first line of antioxidant system (increasing SOD activity and inhibition of catalase) in combination with more intensive work of second line of antioxidant enzymes (significant activation of GPO).
Summary: clinico-biochemical analisis has shown that nonspecific adaptational changes have formed at teenagers that can be included into laboratory algorithms of diagnostic of AH.
Purpose: To examine the state of the vaginal flora in women of late reproductive age with hysteromyoma.
Materials and Methods: The study included 40 women aged 35 to 44 years diagnosed with hysteromyoma. All patients performed a microscopic and bacteriological examination of the vaginal flora.
Results: In patients with hysteromyoma into the sheath showed a reduction in the number of lactobacilli, bifidobacteria and increase the detection rate and the number of nonclostridial anaerobic bacteria.
Summary: hysteromyoma accompanied by changes in vaginal microflora.
Purpose. To study the effects of acute somatic pain on the activity of lysozyme in the blood of animals.
Materials and methods. Experiments were conducted on white adult non-linear male rats. Acute somatic pain was simulated by electrocutaneous stimulation of the receptor zone of the root of the tail. The animals were decapitated and blood sampling was carried out to study the activity of lysozyme by nephelometric method after the acute somatic pain.
Results. Quantity of lysozyme increased in 2 minutes and in 30 minutes after acute somatic pain. Level of lysozyme decreased to the baseline values in 60 minutes after painful stimulation. Statistically significant decline of its level was observed.
Summary. Lysozyme gets involved in the algogenic process when acute pain is experienced. The reaction of lysozyme to the acute somatic pain is transient and biphasic: a rapid increase in the peripheral blood with subsequent decline below the initial values.
Purpose: Analysis of the identified congenital malformations of the fetus, as well as identifying opportunities for their prenatal diagnosis.
Materials and methods: In a medical and social audit, analysis of data, examine the anamnesis approaches for pregnancy, birth, and fetal outcomes among 204 women who gave birth with defects of development. Given the dynamics of growth of malformations for the years 2010-2012 based on the maternity hospital No. 1 of Rostov-on-Don.
Results: There were different types of stains. The work is given the structure of the identified defects. Noted that out of 204 women, only 9 (4.41%) during pregnancy, fetal abnormalities have been identified, the remaining 195 (95.59%) patients vices have not been diagnosed. Such defects have been diagnosed as lumbar hernia, abnormalities of the cardiovascular system and intestines. Other malformations, including chromosomal disease, were not identified. Untimely

Purpose: Assessment of results of surgical treatment of patients with chronic anal cracks with violations of the psychoemotional sphere in the form of alarm and a depression, revealed by means of a hospital scale of alarm and HADS depression.
Materials and methods: The hospital scale of HADS in the preoperative and postoperative period was used, the locking device of a rectum in the preoperative period for definition of a quantitative assessment of expressiveness sphincter spasm was investigated. In work results of surgical treatment of 98 patients were analyzed.
Results: Psychopharmacotherapy application in the postoperative period for the purpose of correction of disturbing and depressive violations is recommended.
Summary: The offered way of treatment of a chronic anal crack taking into account changes of the psychoemotional status and their correction in a complex with surgical and conservative treatment in the postoperative period leads to improvement of subjective health of patients, allows to influence results of surgical treatment, reducing terms of healing of a postoperative wound, reducing risk of emergence of recurrence of surgical treatment of such patients.
Purpose: To identify factors influencing adherence to therapy in HIV-positive injecting drug users and offer a basic conceptual model aimed at improving adherence.
Materials and Methods: Using the method of anonymous questionnaire to assess adherence to therapy in HIV-positive injecting drug users.
Results: the factors that affect adherence to treatment, the study of which is necessary for the development of a conceptual model, which is the basis for the planned activities aimed at improving adherence to treatment in HIV-positive drug users.
Summary: The individual and personal, health, social and organizational factors are involved in the structure of the conceptual model.
Purpose: it was researched the influence of the general infrared sauna on the oxygen transport function of a blood of practical healthy persons.
Materials and methods: analysis of figures of gasotransport blood functions of 77 volunteers before and after the course of total infrared radiation.
Results: it was revealed that graduated thermal loads have training character and are accompanied by change of adaptive reactions. In the acute period of heart stress is stress processes of anaerobic Glycolysis and glycolythichesis oxidoreducsis. After full course of treatment is activated oxygen metabolism and increases functional - metabolic activity mitochondrial enzymes of leucocytes.
Summary: the obtained results are allowed to recommend this method of influence for special contingent of persons with the purpose to increase the level of endurance and resistance the hypoxemic factor at the high temperatures.
Purpose: the estimation of an epidemic situation on a tuberculosis in the south of Russia: in Southern and North Caucasian federal districts; revealing of the basic problems; definition of priority directions of the organization of struggle against a tuberculosis.
Materials and methods: the comparative retrospective analysis of the basic epidemic parameters on a tuberculosis in subjects SFD and NCFD for the period 2005 – is lead(carried out) 2011.
Results: for the period since 2005 on 2011 the epidemic situation in the south of Russia has improved, that proves to be true decrease(reduction) in parameters of desease, morbidity and death rate from a tuberculosis. The basic problems now are insufficient scope of the population by preventive inspections on a tuberculosis and researches on medicinal stability МВТ to antitubercular preparations, and also insufficient security фтизиатрической services the diagnostic equipment.
Summary: Priority directions of the organization of struggle against a tuberculosis in the south of Russia are improvement of quality of early revealing and diagnostics of a tuberculosis with application of preparation Diaskintest ® and preventive photoroentgenography; maintenance of antitubercular establishments with devices Bactec for express train - diagnostics of a tuberculosis and definition of medicinal stability МВТ.
Purpose: To study prostatoprotective activity of burdock thick extracts.
Methods. BPH model reproduced the intraperitoneal administration of male rats sulpiride, 40 mg / kg for 30 days. Thick Burdock extract at a dose of 75 mg / kg and reference drug prostaplant forte, 35 mg / kg was administered intraperitoneally in a medical mode 31 to day 52 of the study.
Results. Mechanism of burdock thick extracts action on BPH is their ability to inhibit lipid peroxidation, normalization ratio of androgens and estrogens inhibit the transition of testosterone to dihydrotestosterone, restore functional capacity and reduce inflammation in the prostate.
Summary. On the severity of prostatoprotective action the thick extracts of roots and leaves of burdock in a dose of 75 mg / kg did not yield to the reference drug prostatoplantu forte.
EXPERIENCE EXCHANGE
Chronic diseases are the most common representatives in the general structure of joints diseases. The aim was to determine the stages of joint damage over time, the specificity of X-ray and ultrasonic methods in the recognition of early and late manifestations of post-traumatic states, degenerative lesions and rheumatoid arthritis. Identified diagnostic and prognostic criteria of joint damage this group of diseases, defined informative and diagnostic significance of radiographic, ultrasonic diagnosis in the early and later stages of the disease process.
In any branch of modern economy the most important factor of production is work. In the last decades work specializes in the health care sphere more and more. The role of laboratory assistants, technicians, technologists, engineers, economists, managers and financiers considerably increases in medical institutions. Features of work in health care are connected with specifics of made «product». So, despite rapid development of new technologies in medicine, replacement of work with the capital here is possible only in limited limits. It is a classical example of branch where work, i.e. «the human capital» and the capital material act rather as complements, than substitutes. For this reason the most important strategic component of development of health care is optimization of management by personnel resources.
An attempt to describe some tactical and technical particularities of the new approach of permanent endocardial lead extraction has been made. Extracorporal circulation is used for direct visual control of targeted leads. Additional recommendations for clinical application of new surgical technique are formulated. High efficacy, reproducibility and low rate of complications have been confirmed.
