Целью обзора является систематический анализ роли нетоза в патогенезе респираторных заболеваний и оценка потенциальных терапевтических стратегий. Рассмотрены механизмы нетоза (литический и витальный нетоз), двойственная функция НВЛ — от антимикробной защиты до медиатора тканевого повреждения. Особое внимание уделено нетозу при COVID-19, бронхиальной астме (в особенности нейтрофильные фенотипы), хронической обструктивной болезни легких и муковисцидозе. Обсуждаются подходы к модуляции и ингибированию нетоза: ДНКаза I, ингибиторы протеаз, ингибиторы фосфодиэстеразы 4 типа, дипептидилпептидазы-1, биологическая терапия. Анализ 55 публикаций (PubMed, e-Library) подтверждает, что нетоз — ключевое патогенетическое звено, а не только биомаркер. Это делает НВЛ перспективной мишенью для лечения, несмотря на гетерогенность заболеваний и сложность регуляции нетоза. Проанализированы результаты исследований по обнаружению, визуализации нейтрофильных внеклеточных ловушек, механизмам ловушкообразования в патогенезе заболеваний органов дыхания, моделированию на лабораторных животных патологических состояний с последующим применением таргетной терапии в отношении компонентов НВЛ.</p></abstract><trans-abstract xml:lang="en"><p>Neutrophil extracellular traps (NETs) are mesh-like structures composed of DNA and antimicrobial proteins released by activated neutrophils during NETosis. This review aims to systematically analyze the role of NETosis in the pathogenesis of respiratory diseases and evaluate potential therapeutic strategies. We discuss the mechanisms of NETosis (lytic and vital), the dual function of NETs — ranging from antimicrobial defense to mediating tissue damage — as well as current detection. Particular emphasis is placed on the role of NETosis in COVID-19, bronchial asthma (specifically neutrophilic phenotypes), chronic obstructive pulmonary disease (COPD), and cystic fibrosis. Therapeutic approaches targeting the modulation and inhibition of NETosis are discussed, including DNase I, protease inhibitors, phosphodiesterase-4 (PDE4) inhibitors, dipeptidyl peptidase-1 (DPP-1) inhibitors, and biological therapies. Analysis of 55 publications retrieved from PubMed and eLibrary confirms that NETosis is a central pathogenetic mechanism rather than solely a biomarker. This positions NETs as a promising therapeutic target, despite disease heterogeneity and the complexity of NETosis regulation. The review also summarizes research findings on NET detection and visualization, mechanisms of NET formation in respiratory pathology, animal modeling of disease states, and the subsequent application of targeted therapies directed against NET components.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>обзор</kwd><kwd>нетоз</kwd><kwd>нейтрофильные внеклеточные ловушки (НВЛ)</kwd><kwd>нейтрофилы</kwd><kwd>респираторные заболевания</kwd><kwd>таргетная терапия</kwd></kwd-group><kwd-group xml:lang="en"><kwd>review</kwd><kwd>NETosis</kwd><kwd>neutrophil extracellular traps (NETs)</kwd><kwd>neutrophils</kwd><kwd>respiratory diseases</kwd><kwd>targeted therapy</kwd></kwd-group></article-meta></front><body><sec><title>Введение</title><p>Хронические воспалительные заболевания органов дыхания (бронхиальная астма — БА, хроническая обструктивная болезнь легких — ХОБЛ, муковисцидоз — МВ) остаются ведущими причинами заболеваемости и смертности. В основе патогенеза — нарушение иммунного ответа, ключевые эффекторы которого — нейтрофилы. Традиционно рассматриваемые как первая линия защиты, при определенных условиях они усугубляют повреждение тканей [<xref ref-type="bibr" rid="cit1">1</xref>]. Открытие в 2004 г. нетоза изменило понимание роли нейтрофилов в воспалении [<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>Продукт нетоза — нейтрофильные внеклеточные ловушки (Neutrophil Extracellular Traps, NETs или НВЛ) — внеклеточные сети из деконденсированной ДНК, гистонов и гранулярных белков [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit3">3</xref>]. При острой инфекции НВЛ нейтрализуют патогены. При хроническом воспалении избыточное ловушкообразование повреждает эпителий, повышает вязкость секрета, активирует тромбообразование и поддерживает воспаление [<xref ref-type="bibr" rid="cit3">3</xref>].</p><p>Стандартная терапия часто неэффективна при нейтрофильных фенотипах. Селективная модуляция нетоза открывает перспективы лечения респираторных заболеваний.</p><p>Поиск литературы проводился в базах данных PubMed и eLibrary за период с 2006 по август 2025 г., включающий 110 источников. Основу анализа составили научные публикации за последние 5 лет (2020–2025 гг.). В качестве исключения в обзор были включены основополагающие работы, датированные 2006–2019 гг. и имеющие принципиальное значение для понимания механизмов образования нейтрофильных внеклеточных ловушек, эволюции представлений о нетозе и становления данного направления в патофизиологии респираторных заболеваний. Критериями включения служили оригинальные исследования, обзоры и метаанализы на русском и английском языках; критериями исключения — тезисы конференций, диссертации, статьи без полнотекстового доступа. Поиск осуществлялся по ключевым словам: «нейтрофильные внеклеточные ловушки», «нетоз», «neutrophil extracellular traps», «NETosis», «нейтрофилы», «респираторные заболевания», «таргетная терапия». После окончательного отбора источников литературы в анализ включено 55 источников.</p></sec><sec><title>Общая характеристика и механизм нетоза</title><p>Основная клетка, реализующая механизм нетоза — нейтрофильный гранулоцит. Нейтрофил — это полиморфноядерный лейкоцит миелоидного ряда, представляющий собой основной эффектор врождённого клеточного иммунитета; характеризуется коротким жизненным циклом. Нейтрофилы реализуют антимикробную защиту через дегрануляцию, фагоцитоз и нетоз — процесс выброса внеклеточных сетчатых структур, состоящих из деконденсированной ДНК, гистонов и гранулярных белков: нейтрофильной эластазы (НЭ), миелопероксидазы (МПО), протеиназы 3 [<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit6">6</xref>].</p><p>Ультраструктурно НВЛ представляют собой филаменты с глобулярными доменами, содержащими гранулярные белки [<xref ref-type="bibr" rid="cit4">4</xref>]. При активации in vitro объём сетеподобной ловушки [<xref ref-type="bibr" rid="cit1">1</xref>][<xref ref-type="bibr" rid="cit2">2</xref>], может превышать клеточный в 10–15 раз, полностью заполняя альвеолярное пространство [<xref ref-type="bibr" rid="cit4">4</xref>].</p><p>Образование НВЛ протекает по двум основным путям. Литический (суицидальный, классический) нетоз завершается гибелью нейтрофила и длится от 2 до 4 часов [<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit8">8</xref>][<xref ref-type="bibr" rid="cit9">9</xref>]. Мощным индуктором in vitro является форбол-12-миристат-13-ацетат (ФМА), активирующий протеинкиназу C и вызывающий потерю сегментации ядра, деконденсацию хроматина и разрыв ядерной и плазматической мембран [<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit8">8</xref>][<xref ref-type="bibr" rid="cit9">9</xref>]. Запуск суицидального нетоза сопровождается активацией НАДФН-оксидазы, способствуя генерации активных форм кислорода (АФК) [<xref ref-type="bibr" rid="cit10">10</xref>]. АФК обеспечивают транслокацию НЭ и МПО в ядро, где они расщепляют гистоны и усиливают их деконденсацию; параллельно пептидиларгининдезиминаза 4 (ПАД4) цитруллинирует гистон H3, ослабляя его связь с ДНК, а газдермин D формирует поры в мембранах, вызывая лизис клетки [<xref ref-type="bibr" rid="cit11">11</xref>]. Нелитический (витальный) нетоз развивается за 5–60 минут без гибели клетки. Хроматин и белки секретируются экзоцитозом в виде везикул, при этом нейтрофил сохраняет способность к хемотаксису и фагоцитозу [<xref ref-type="bibr" rid="cit12">12</xref>][<xref ref-type="bibr" rid="cit13">13</xref>]. Этот путь наблюдается при взаимодействии с активированными тромбоцитами или при инфекции, вызванной S. aureus и C. albicans [<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit8">8</xref>]. Оба пути требуют адгезии нейтрофила к субстрату через интегрины, что объясняет отсутствие спонтанного образования НВЛ в кровотоке. Также описан митохондриальный нетоз — вариант витального пути с выделением митохондриальной ДНК [<xref ref-type="bibr" rid="cit9">9</xref>].</p><p>Молекулярное распознавание патогенов при нетозе опосредовано преимущественно толл-подобными рецепторами (ТПР или Toll-like receptors, TLR): ТПР2 активируются пептидогликанами грамположительных бактерий и компонентами грибов, ТПР4 — липополисахаридами (ЛПС) грамотрицательных бактерий [<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit9">9</xref>]. Их стимуляция запускает сигнальные каскады (PKC, Src-киназы, Ras/Raf/MEK/ERK) [<xref ref-type="bibr" rid="cit14">14</xref>]. Клинически значимо, что полиморфизмы генов ТПР1 и ТПР4, ассоциированных со сниженной активностью нетоза, коррелируют с повышенной смертностью при пневмонии [<xref ref-type="bibr" rid="cit15">15</xref>].</p><p>Регуляция нетоза зависит от множества триггеров. Ключевыми индукторами образования НВЛ могут быть инфекционные (бактериальные, вирусные и грибковые) и неинфекционные (химические митогены, аутоиммунные и иммунные, онкологические и другие — моноурат натрия при подагре, сигаретный дым) агенты [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit8">8</xref>][<xref ref-type="bibr" rid="cit9">9</xref>].</p><p>Нетоз реализует две биологические стратегии. С одной стороны, НВЛ эффективно улавливают крупные патогены, например, грибковые гифы. Мощное микробицидное действие обеспечивается за счёт цитруллинированных гистонов и НЭ, МПО, АФК, ДНК [<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit9">9</xref>].</p><p>С другой стороны, при хроническом воспалении компоненты НВЛ становятся источником молекулярных паттернов, связанных с повреждением (Damage-Associated Molecular Patterns, DAMPs). Данные молекулы активируют cGAS–STING-путь, повышают проницаемость эндотелия и способствуют развитию иммунотромбоза — микротромбов в капиллярах лёгких при острых респираторных дистресс-синдромах [<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit9">9</xref>]. Соответственно, повышенные уровни цитруллинированного гистона H3 (CitH3), внеклеточной ДНК и комплексов миелопероксидаза–ДНК (МПО–ДНК) положительно коррелируют с тяжестью таких заболеваний, как астма, ХОБЛ, ОРДС и COVID-19 [<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit8">8</xref>].</p><p>Некоторые патогены выработали механизмы эвазии от НВЛ («ускользания от распознавания»). Streptococcus pneumoniae, Staphylococcus aureus, Pseudomonas aeruginosa продуцируют ДНКазы, расщепляющие ДНК-остов ловушек и позволяющие микроорганизмам «вырываться» из сети. Bordetella pertussis подавляет нетоз путём повышения внутриклеточного уровня циклического АМФ (цАМФ). Aspergillus fumigatus маскируется от распознавания с помощью гидрофобина RodA, нейтрализующего действие НВЛ [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit9">9</xref>].</p><p>Отдельные исследования показывают, что компенсаторные кислородзависимые механизмы активируются при воздействии бактерий даже в больших концентрациях.  При воздействии новыми классами соединений, например, наночастицами магнетита (НЧМ) в сублетальных концентрациях происходил «срыв» работы НАДФН-оксидазы, но активность МПО сохранялась. Это указывает на сложность регуляции «дыхательного взрыва» в нейтрофилах и подтверждает двойственность нетоза [<xref ref-type="bibr" rid="cit16">16</xref>].</p></sec><sec><title>Микроскопические методы выявления НВЛ в научных и клинических целях</title><p>Для изучения морфологии предпочтительны суспензии из бронхоальвеолярного лаважа (БАЛ) или периферической крови [<xref ref-type="bibr" rid="cit5">5</xref>]. Методы: световая, флуоресцентная, электронная микроскопия. При классическом нетозе сканирующая электронная микроскопия (СЭМ) выявляет разветвлённые ДНК-нити, при витальном — «бусы на нити». Флуоресцентная микроскопия (антитела к МПО, CitH3), осуществляемая иммуногистохимическим методом, различает компоненты ловушек [<xref ref-type="bibr" rid="cit4">4</xref>]. Для клинических лабораторий доступен метод световой микроскопии с двойным окрашиванием: гексаазотированный фуксин выявляет НЭ, метиловый зелёный — ДНК [<xref ref-type="bibr" rid="cit17">17</xref>][<xref ref-type="bibr" rid="cit18">18</xref>].</p></sec><sec><title>Нетоз в патогенезе заболеваний лёгких</title><p>Механизм нетоза описан как при острых, так и при хронических состояниях лёгких, особенно при тяжёлом повреждении паренхимы, сопровождающимся массивной нейтрофильной инфильтрацией и деструкцией альвеолярно-капиллярного барьера. Интерес к этой форме иммунного ответа резко возрос в период пандемии COVID-19, когда было доказано, что чрезмерный нетоз способствует развитию микротромбозов, «цитокинового шторма» и прогрессированию острого респираторного дистресс-синдрома (ОРДС), выступая не только как защитная реакция, но и как центральное звено патогенеза.</p><p>При COVID-19 инфекция SARS-CoV-2 напрямую активирует нетоз через повышение АФК и путь протеинкиназы C-β (ПКC-β) [<xref ref-type="bibr" rid="cit19">19</xref>][<xref ref-type="bibr" rid="cit20">20</xref>]. ЛПС-индуцированный нетоз подавляется ингибитором ПКC-β рубоксистаурином in vitro без угнетения фагоцитоза, что открывает терапевтические перспективы [<xref ref-type="bibr" rid="cit20">20</xref>]. Уровни маркеров нетоза (внеклеточная ДНК, комплексы МПО-ДНК, CitH3) положительно коррелируют с нейтрофилезом, D-димером и тяжестью заболевания, включая риск ОРДС и летальный исход [<xref ref-type="bibr" rid="cit21">21</xref>].</p><p>Частной моделью ОРДС является синдром TRALI («острое повреждение лёгких, связанное с трансфузией»), сопровождающийся массивной инфильтрацией нейтрофилов и ловушкообразованием [<xref ref-type="bibr" rid="cit22">22</xref>]. Критическую роль играет взаимодействие нейтрофилов и тромбоцитов, создающее порочный круг иммунотромбоза [<xref ref-type="bibr" rid="cit23">23</xref>]. Экспериментальное блокирование компонентов НВЛ (ДНКаза I, антигистоновые антитела) или ингибирование активации тромбоцитов уменьшает отёк лёгких и смертность [<xref ref-type="bibr" rid="cit24">24</xref>].</p><p>При нейтрофильной бронхиальной астме (не-Th2-астме, стероидорезистентной, ассоциированной с ожирением) центральным звеном является нетоз, способствующий повреждению дыхательных путей и нарушению иммунной регуляции.</p><p>Индукция нетоза запускается ЛПС через активацию TПР4 и транскрипцию ИЛ-1β, ФНО-α, CXCL8 [<xref ref-type="bibr" rid="cit25">25</xref>][<xref ref-type="bibr" rid="cit26">26</xref>]. ИЛ-8 через рецептор CXCR2 инициирует нетоз и накопление нейтрофилов [<xref ref-type="bibr" rid="cit27">27</xref>]. НВЛ активируют NLRP3-инфламмасому, усиливая созревание ИЛ-1β и продукцию ИЛ-17A [<xref ref-type="bibr" rid="cit26">26</xref>]. ИЛ-17A стимулирует секрецию CXCL8, замыкая «порочный круг», и индуцирует высвобождение НЭ и матриксной металлопротеиназы-9 (ММП-9), вызывая ремоделирование [<xref ref-type="bibr" rid="cit26">26</xref>].</p><p>Сигаретный дым усиливает инфильтрацию и стимулирует нетоз через ИЛ-33/ST2 (аутофагия, mTOR–Hif-1α) и снижение ИЛ-35 (ось gp130/STAT3, ферроптоз) [<xref ref-type="bibr" rid="cit28">28</xref>][<xref ref-type="bibr" rid="cit29">29</xref>][<xref ref-type="bibr" rid="cit30">30</xref>]. Воспаление устойчиво к глюкокортикостероидам (ГКС): они подавляют апоптоз нейтрофилов, увеличивая выживаемость и потенциал к нетозу [<xref ref-type="bibr" rid="cit26">26</xref>][<xref ref-type="bibr" rid="cit30">30</xref>].</p><p>При астме, ассоциированной с ожирением, пальмитиновая кислота, резистин и лептин активируют ТПР4, запуская синтез ИЛ-1β и ИЛ-18 [<xref ref-type="bibr" rid="cit25">25</xref>]. Цитокины стимулируют ILC3 (лимфоидные клетки врождённого иммунитета 3-го типа) и Th17-клетки (Т-хелперы 17) к продукции ИЛ-17A. Отмечено повышение маркеров нейтрофилов (Gr-1) и НВЛ, коррелирующее с гиперреактивностью [<xref ref-type="bibr" rid="cit25">25</xref>].</p><p>Критический этап — цитруллинирование гистонов. При тяжёлой астме повышены уровни ПАД4 и CitH3 [<xref ref-type="bibr" rid="cit31">31</xref>][<xref ref-type="bibr" rid="cit32">32</xref>]. Симвастатин подавляет экспрессию ПАД4, снижая образование НВЛ [<xref ref-type="bibr" rid="cit31">31</xref>].</p><p>НВЛ оказывают цитотоксичность через повреждение мембран гистонами, активируют макрофаги M1, а также индуцируют экспрессию тимусного стромального лимфопоэтина (TSLP), матриксной металлоппротеиназы-9 (ММП-9), фактора роста эндотелия сосудов (VEGF) [<xref ref-type="bibr" rid="cit28">28</xref>][<xref ref-type="bibr" rid="cit33">33</xref>].</p><p>Разработаны следующие терапевтические стратегии. Ингибирование ПАД4 (Cl-amidin, симвастатин, ингибиторы MBD2 и JAK2) снижает образование НВЛ [<xref ref-type="bibr" rid="cit31">31</xref>][<xref ref-type="bibr" rid="cit32">32</xref>]. Блокада ИЛ-33 (sST2) подавляет нетоз при астме, индуцированной дымом [<xref ref-type="bibr" rid="cit28">28</xref>]; экзогенный ИЛ-35 восстанавливает баланс Th17/Treg [<xref ref-type="bibr" rid="cit29">29</xref>]. Инволюкрасин B ингибирует сигнальный путь при астме, связанной с ожирением [<xref ref-type="bibr" rid="cit25">25</xref>]. Антагонисты CXCR2 перспективны для лечения курильщиков, но на данный момент препараты не одобрены для широкого клинического применения [<xref ref-type="bibr" rid="cit30">30</xref>].</p><p>Персонализированный подход потребует идентификации «нетоз-доминантного» эндофенотипа по маркерам (CitH3, комплексы МПО–ДНК).</p><p>Хроническая обструктивная болезнь лёгких (ХОБЛ) — хроническое воспалительное заболевание с необратимым ограничением воздушного потока [<xref ref-type="bibr" rid="cit34">34</xref>][<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit36">36</xref>]. Диагностируется при наличии одышки, хронического кашля и факторов риска (курение) [<xref ref-type="bibr" rid="cit37">37</xref>]. Патогенез обусловлен воздействием дыма, вызывающего нейтрофильное воспаление — ключевой драйвер повреждения [<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit36">36</xref>].</p><p>Роль нетоза преимущественно негативна. Уровень НВЛ в мокроте коррелирует с тяжестью, функцией лёгких и неэозинофильными обострениями (Haemophilus influenzae) [<xref ref-type="bibr" rid="cit35">35</xref>]. НВЛ подавляют фагоцитоз, объясняя рецидивирующие инфекции [<xref ref-type="bibr" rid="cit35">35</xref>]. Повреждение реализуется через действие протеаз: разрушают альвеолы и стимулируют гиперсекрецию слизи [<xref ref-type="bibr" rid="cit38">38</xref>][<xref ref-type="bibr" rid="cit39">39</xref>]. Нейтрофилёз в крови и БАЛ связан с летальностью [<xref ref-type="bibr" rid="cit40">40</xref>]. Чрезмерная активность нейтрофилов способствует ремоделированию и обострениям [<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit36">36</xref>].</p><p>Выделяют следующие фенотипы: Тh2 (алармины) и нейтрофильный (устойчив к ГКС, цитокины: ИЛ-8, ФНО-α, ИЛ-17, ИЛ-23) [<xref ref-type="bibr" rid="cit34">34</xref>].</p><p>Пиоглитазон и розиглитазон проявляют высокое сродство к ключевым маркерам нетоза при обострениях ХОБЛ, вызванных P. aeruginosa: азуроцидину 1 (антимикробный хемотаксический белок гранул нейтрофилов), миелопероксидазе (фермент, генерирующий бактерицидные АФК) и резистину (провоспалительный цитокин с метаболической активностью), экспрессия которых коррелирует с выраженностью инфекционного процесса [<xref ref-type="bibr" rid="cit41">41</xref>]. Moraxella osloensis поддерживает воспаление и усиленное образование НВЛ через активацию TLR4, что также обусловливает тяжесть заболевания [<xref ref-type="bibr" rid="cit42">42</xref>].</p><p>Муковисцидоз (МВ) — ведущая причина развития бронхоэктазов, характеризующихся тяжёлым хроническим гнойно-воспалительным поражением дыхательных путей. В основе дисфункция белка CFTR (трансмембранного регулятора), кодирующего хлоридно-бикарбонатный канал [<xref ref-type="bibr" rid="cit43">43</xref>]. Однако прогрессирование заболевания обусловлено не генетическим дефектом, а его следствием — воспалением. Нарушение ионного транспорта приводит к снижению мукоцилиарного клиренса и персистенции бактериальной инфекции [<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit43">43</xref>][<xref ref-type="bibr" rid="cit44">44</xref>][<xref ref-type="bibr" rid="cit45">45</xref>][<xref ref-type="bibr" rid="cit46">46</xref>].</p><p>Протеомика мокроты показывает корреляцию маркеров нетоза (НЭ, МПО, CitH3, ДНК) с тяжестью и ответом на терапию [<xref ref-type="bibr" rid="cit47">47</xref>].</p><p>При МВ колонизация респираторного тракта P. aeruginosa ухудшает функцию дыхания [<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit45">45</xref>]. Дыхательные пути инфильтрированы нейтрофилами [<xref ref-type="bibr" rid="cit44">44</xref>][<xref ref-type="bibr" rid="cit45">45</xref>][<xref ref-type="bibr" rid="cit46">46</xref>][<xref ref-type="bibr" rid="cit48">48</xref>][<xref ref-type="bibr" rid="cit49">49</xref>][<xref ref-type="bibr" rid="cit50">50</xref>]. Нейтрофилы не контролируют инфекцию из-за дисфункции и прибегают к нетозу [<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit43">43</xref>][<xref ref-type="bibr" rid="cit44">44</xref>]. Уровни НВЛ коррелируют с медиаторами (IL-8, НЭ) и тяжестью заболевания [<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit44">44</xref>][<xref ref-type="bibr" rid="cit51">51</xref>]. Эффективность НВЛ ограничена: P. aeruginosa выработала механизмы уклонения (биопленки, сиаловые кислоты) [<xref ref-type="bibr" rid="cit35">35</xref>], которые обеспечивают толерантность к НВЛ. Избыточное ловушкообразование усугубляет обструкцию вследствие поддержания воспаления [<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit43">43</xref>][<xref ref-type="bibr" rid="cit44">44</xref>][<xref ref-type="bibr" rid="cit51">51</xref>].</p></sec><sec><title>Стратегии таргетного лечения, подавляющего нетоз и повреждающее действие компонентов НВЛ</title></sec><sec><title>Ингибиторы протеаз (ингибиторы нейтрофильной эластазы)</title><p>НЭ способствует тканевому повреждению, гиперреактивности дыхательных путей, ремоделированию эпителия и формированию НВЛ. Несмотря на многообещающие данные доклинических исследований, клинические испытания прямых ингибиторов НЭ (AZD9668, BAY 85-8501) при МВ, ХОБЛ и бронхоэктазах не продемонстрировали значимого улучшения лабораторной или клинической динамики [<xref ref-type="bibr" rid="cit52">52</xref>]. Это свидетельствует о том, что изолированное подавление внеклеточной НЭ недостаточно для контроля сложного воспалительного каскада данных нозологий.</p></sec><sec><title>Дезоксирибонуклеаза I (ДНКаза I)</title><p>ДНКазное лечение направлено на деградацию внеклеточной ДНК, которая служит «каркасом» НВЛ и способствует повышению вязкости бронхиального секрета. ДНКаза I обладает благоприятным профилем безопасности и рассматривается как терапевтический агент при заболеваниях дыхательной системы, ассоциированных с нетозом. При ОРДС (включая TRALI, VILI, сепсис- и SARS-CoV-2-индуцированные формы [<xref ref-type="bibr" rid="cit21">21</xref>]) она снижает накопление НВЛ, нейтрофильную инфильтрацию, повреждение лёгких и смертность в экспериментальных моделях [<xref ref-type="bibr" rid="cit24">24</xref>]. На молекулярном уровне терапия сопровождалась снижением активности МПО, уровней CitH3 и НЭ, уменьшением концентраций провоспалительных цитокинов (ИЛ-1β, ИЛ-6 и ФНО-α) [<xref ref-type="bibr" rid="cit24">24</xref>]. Однако действие ДНКазы I ограничено: препарат не устраняет полностью белковые компоненты НВЛ. В некоторых исследованиях ДНКаза I не оказывала влияния на уровень цитокинов, а также было установлено, что она обладает коротким периодом полураспада, затрудняющим поддержание её терапевтической концентрации в системном кровотоке.</p><p>Ингаляционная терапия дорназой альфа (рекомбинантная ДНКаза I) при МВ расщепляет внеклеточную ДНК, уменьшая вязкость мокроты и риск обострений. Лечение снижает уровень НВЛ и их провоспалительный потенциал, что указывает на противовоспалительную роль препарата помимо муколитического эффекта [<xref ref-type="bibr" rid="cit44">44</xref>]. Однако при бронхоэктазах наблюдается отрицательная динамика: распад НВЛ сопровождается высвобождением активных протеаз (НЭ, протеаза 3, катепсин G), усугубляющим тканевое повреждение [<xref ref-type="bibr" rid="cit52">52</xref>].</p><p>Дальнейшие исследования должны быть направлены на оптимизацию фармакокинетики, а также на комбинированные подходы, сочетающие деградацию ДНК-остова НВЛ с ингибированием протеаз нетоза.</p></sec><sec><title>Ингибиторы фосфодиэстеразы 4 типа (ФДЭ4)</title><p>Рофлумиласт эффективен при МВ и ХОБЛ. Препарат подавляет ключевые звенья нетоза in vitro и in vivo. Блокада ФДЭ4 ограничивает высвобождение свободной ДНК и снижает уровень CitH3 за счёт подавления активности ПАД4, сохраняя целостность нейтрофилов и не нарушая клиренс бактерий. В модели хронической инфекции P. aeruginosa у мышей отмечено уменьшение нейтрофильного воспаления [<xref ref-type="bibr" rid="cit53">53</xref>]. При ХОБЛ рофлумиласт одобрен для снижения риска обострений: терапевтический эффект опосредован повышением внутриклеточного цАМФ, что подавляет активацию иммунных клеток и высвобождение провоспалительных медиаторов. Однако клиническое применение ограничено побочными эффектами системного действия.</p><p>Двойственная роль нетоза — защитная при острой инфекции и патогенная при избыточной активации — определяет перспективные направления терапии. ДНКаза I, ингибиторы ПАД4 (Cl-amidin, GSK484) и селективные ингибиторы НЭ (сивелестат) снижают воспаление и улучшают оксигенацию в моделях острых лёгочных повреждений [<xref ref-type="bibr" rid="cit7">7</xref>]. Большинство исследований проводится in vitro с использованием нефизиологических стимулов (ФМА) [<xref ref-type="bibr" rid="cit8">8</xref>], и дальнейшие работы необходимы для верификации механизмов нетоза в условиях, имитирующих патофизиологию человека.</p></sec><sec><title>Ингибитор дипептидилпептидазы 1 (ДПП-1)</title><p>Новый препарат бренсокатиб показал эффективность в отношении нейтрофильного воспаления при бронхоэктазах [<xref ref-type="bibr" rid="cit54">54</xref>]. Бренсокатиб — пероральный обратимый ингибитор дипептидилпептидазы-1 (ДПП-1), подавляющий активацию сериновых протеаз нейтрофилов. Уникальная способность бренсокатиба воздействовать на нейтрофильную активность может послужить основой для разработки подходов к изучению роли нетоза в прогрессировании заболевания и стратегий лечения.</p></sec><sec><title>Биологическая терапия</title><p>Гуманизированное моноклональное антитело экулизумаб ингибирует протеолитическое расщепление компонента комплемента C5, предотвращая образование анафилатоксина C5a, и блокирует формирование мембраноатакующего комплекса. Данные эффекты предотвращают высвобождение компонентов НВЛ. Препарат не применяется в рутинной практике для лечения респираторных заболеваний, но его механизм действия представляет значительный научный интерес и требует изучения для разработки таргетных стратегий модуляции НВЛ-опосредованного воспаления при патологии дыхательной системы [<xref ref-type="bibr" rid="cit55">55</xref>].</p></sec><sec><title>Заключение</title><p>Физиологический нетоз выполняет защитную функцию, обеспечивая захват патогенов и предотвращение их диссеминации, однако избыточная активация процесса способствует тканевому повреждению, ремоделированию дыхательных путей и поддержанию воспаления. Патогенетическая значимость подтверждена при COVID-19 и ОРДС (микротромбозы, «цитокиновый шторм»), а также при нейтрофильных фенотипах БА, ХОБЛ и МВ, где активность коррелирует с тяжестью и стероидорезистентностью. Регуляция определяется различными сигнальными путями и цитокинами. Перспективные стратегии: ингибиторы ПАД4, ДНКаза I, ингибиторы ФДЭ4 и ДПП-1, биологическая терапия. Несмотря на результаты in vitro, многие препараты не доказали эффективности у пациентов из-за гетерогенности эндофенотипов. Необходима персонализация лечения с выявлением «нетоз-доминантного» профиля по маркерам НВЛ.
