В настоящее время отечественных публикаций, рассматривающих вопросы классификации, клиники, диагностики, лечения мелкоклеточного рака, одной из форм рака лёгкого, крайне мало. Представленный обзор литературы посвящён мелкоклеточному раку лёгкого, опухоли, имеющей агрессивное клиническое течение и неблагоприятный прогноз. В обзоре рассмотрены вопросы эпидемиологии, клинической и морфологической классификации мелкоклеточного рака лёгкого, рекомендуется наиболее приемлемая клиническая классификация, которая подразделяет опухоль на две формы (локальную, или ограниченную, и распространённую). В работе уделяется внимание стандартным вариантам лечения локальной и распространённой форм мелкоклеточного рака лёгкого. В лечении локальной формы подтверждена основная роль химиотерапии с лучевой терапией, комбинированной химиотерапии, хирургического лечения с последующей химиотерапией или химиолучевым лечением. Для лечения распространённой формы указаны такие варианты лечения, как комбинированная химиотерапия и лучевая терапия. Отмечены успехи и недостатки в лечении этой опухоли полихимиотерапией, лучевой терапией и комбинированной терапией, а также указывается на разработку новых схем и комбинаций лечения с большей эффективностью и меньшей токсичностью для больных. Отдельно рассмотрены вопросы терапии рецидивов мелкоклеточного рака лёгкого, отмечена основная роль в лечении химиотерапии и паллиативного лечения. Показано значение облучения головного мозга в плане снижения рецидивирования и увеличения продолжительности жизни при этой патологии</p></abstract><trans-abstract xml:lang="en"><p>Lung cancer – one of the most common malignancies in the world. Currently, domestic publications addressing issues of classifi cation, clinic, diagnosis, treatment, a form of lung cancer - non-small cell cancer, is extremely small. Th erefore, our work makes up to some extent this gap. Th is review of literature devoted to small-cell lung cancer, the tumor has an aggressive clinical course and poor prognosis. Th e review considers the epidemiology, clinical and morphological classifi cation of small cell lung cancer; it is recommended the most appropriate clinical classifi cation, which classifi es tumors into two forms — limited or local and extensive. Th e paper focuses on the standard options of treatment of limited form and extensive form of non-small cell lung cancer. In the treatment of limited form confi rmed the primary role of chemotherapy with radiation therapy, combination chemotherapy, surgery followed by chemotherapy or chemoradiotherapy. For the treatment of extensive form shown in the use of such treatment options as combination chemotherapy and radiotherapy. It noted the successes and shortcomings in the treatment of this tumor chemotherapy, radiation therapy, and combination therapy, and also points to the development of new schemes and treatment combinations with greater effi ciency and less toxicity for patients. Separately, the issues of treatment of small cell lung cancer recurrence and noted the major role in the treatment of chemotherapy and palliative care.
