Дифференциальная диагностика синдрома скопления жидкости в брюшной полости у женщин представляет собой сложную клиническую задачу, требующую исключения широкого спектра патологий, включая цирроз печени и его осложнения, в том числе гепатоцеллюлярную карциному, злокачественные новообразования другой локализации, сердечную недостаточность и гинекологические заболевания. Несмотря на совершенствование диагностических методов, сохраняется тенденция к поздней верификации опухолей яичников, что ухудшает прогноз лечения. В работе представлено клиническое наблюдение пациентки (66 лет), поступившей в стационар с клиникой массивного асцита. Проведённый комплекс обследований, включая мультиспиральную компьютерную томографию (МСКТ) органов брюшной полости и малого таза с контрастным усилением, позволил визуализировать объёмное кистозное образование яичника гигантских размеров. Выполнено оперативное лечение — аднексэктомия. Гистологическое заключение: серозная цистаденома (цистаденофиброма). Отмечен положительный терапевтический эффект без рецидива асцита в послеоперационном периоде. Данный случай демонстрирует диагностическую ценность методов лучевой визуализации, в частности МСКТ, в установлении генеза гидроперитонеума у пациенток постменопаузального возраста и подчёркивает необходимость включения гинекологической патологии в дифференциально-диагностический ряд в соответствии с клиническими рекомендациями.

Differential diagnosis of abdominal fluid accumulation syndrome in women is a complex clinical task, requiring exclusion of a wide range of pathologies, including cirrhosis and its complications, including hepatocellular carcinoma, malignant neoplasms of another location, heart failure and gynecological diseases. Despite the improvement of diagnostic methods, there is a tendency towards late verification of ovarian tumors, which worsens the prognosis of treatment. The paper presents a clinical observation of a 66-year-old patient admitted to a hospital with a clinic of massive ascites. A complex of examinations, including multispiral computed tomography (MSCT) of the abdominal and pelvic organs with contrast enhancement, made it possible to visualize the volumetric cystic formation of a giant ovary. Surgical treatment was performed - adnexectomy. Histological conclusion: serous cystadenoma (cystadenofibroma). A positive therapeutic effect was noted without recurrence of ascites in the postoperative period. This case demonstrates the diagnostic value of radiation imaging methods, in particular MSCT, in establishing the genesis of hydroperitoneum in postmenopausal patients and emphasizes the need to include gynecological pathology in the differential diagnostic series in accordance with clinical guidelines.
