Цель: выявить ассоциации инсомнических, депрессивных, суицидальных, тревожных проявлений с выраженностью психопатологической симптоматики у людей с шизофренией.

Материалы и методы

Материалы и методы: было проведено сравнительное кросс-секционное исследование 390 пациентов. В ходе работы были применены шкала PANSS, Питтсбургский индекс качества сна, шкала депрессии Калгари, Колумбийская шкала серьёзности суицидальных намерений, шкала тревоги Гамильтона. Статистический анализ проводился с помощью U-критерия Манна — Уитни, коэффициента ранговой корреляции Спирмена.

Результаты

Результаты: инсомнические нарушения оказались связаны с симптомами Р1, Р3 и общим баллом по субшкале позитивных симптомов; депрессивные проявления были связаны с большей выраженностью всех позитивных и негативных симптомов за исключением Р4, Р5, N5; тревожные проявления — с большей выраженностью симптомов Р3, Р6 и с меньшей выраженностью симптомов N2, N3, N6; интенсивность суицидальных мыслей положительно коррелировала с тяжестью симптомов Р3, Р6, N4, а также с общим баллом по шкале позитивных симптомов; наличие же суицидальной попытки в анамнезе оказалось ассоциировано с более тяжёлыми симптомами Р1, Р3, Р6.

Заключение

Заключение: инсомнические, депрессивные, тревожные проявления и суицидальные феномены оказались связаны с выраженностью психопатологической симптоматики у пациентов с параноидной шизофренией, но характер связей оказался неоднородным. Выраженность инсомнических нарушений положительно коррелировала с выраженностью позитивных симптомов. Депрессивная симптоматика, суицидальные феномены и тревожные проявления были ассоциированы с выраженностью как позитивных, так и негативные симптомов.

Objective

Objective: to identify associations of insomnic, depressive, suicidal, and anxiety manifestations with the severity of psychopathological symptoms in individuals with schizophrenia.

Materials and methods

Materials and methods: a comparative cross-sectional study of 390 patients was conducted. The following scales were used: PANSS, Pittsburgh Sleep Quality Index, Calgary Depression Scale, Columbia Suicide Severity Scale, Hamilton Anxiety Scale. Statistical analysis was performed using the Mann–Whitney U-test and Spearman rank correlation coefficient.

Results

Results: insomnia disorders were associated with symptoms P1, P3 and the total score on the positive symptoms subscale; depressive manifestations were associated with greater severity of all positive and negative symptoms except P4, P5, N5; anxious manifestations - with greater severity of symptoms P3, P6, and with lesser severity of symptoms N2, N3, N6; the intensity of suicidal thoughts intensity positively correlated with the severity of symptoms P3, P6, N4, as well as with the total score on the positive symptoms scale; A history of suicide attempt was associated with more severe symptoms of P1, P3, and P6.

Conclusion

Conclusion: insomnia, depressive, anxiety, and suicidal tendencies were associated with the severity of psychopathological symptoms in patients with paranoid schizophrenia, but the nature of the associations was heterogeneous. The severity of insomnia disorders was positively correlated with the severity of positive symptoms. Depressive symptoms, suicidal tendencies, and anxiety were associated with the severity of both positive and negative symptoms.
