Были исследованы биохимические показатели, уровни инсулина, глюкагона, С-пептида до и после стандартного завтрака, рассчитаны индексы НОМА IR и НОМА β.</p></sec><sec><title>Результаты</title><p>Результаты: мы получили значимую разницу между группами приема иДПП4 более года и Метформином+СМ по уровню глюкагона и инсулина натощак, однако показатели инсулина были в пределах нормы в обеих группах. Кроме того, после 6 месяцев впервые начатой терапии иДПП4 снижались уровни инсулина как до, так и после стандартного завтрака, С-пептида после стандартного завтрака и глюкагона натощак.</p></sec><sec><title>Выводы</title><p>Выводы: полученные данные свидетельствуют о способности иДПП4 положительно влиять на два наиболее ранних и высокозначимых звена патогенеза СД2 типа. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose: evaluate the eff ects of DPP4i on fasting and postprandial insulin and glucagon secretion by examining basal secretion and response to food loading.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: patients (n = 54) were divided into treatment groups: long-term (more than a year) therapy with iDPP4 with Metformin, Metformin + sulfonylurea, fi rst-time therapy with iDPP4. Biochemical parameters, levels of insulin, glucagon, C-peptide before and aft er a standard breakfast were measured. Th e HOMA IR and HOMA β indices were calculated. Results: we obtained a signifi cant diff erence in fasting glucagon and insulin levels between the iDPP4 over a year and Metformin + SM groups. In addition, insulin levels before and aft er standard breakfast, C-peptide aft er standard breakfast, and fasting glucagon decreased aft er 6 months of fi rst-time DPP4 therapy.</p></sec><sec><title>Summary</title><p>Summary: the data obtained indicate the ability of iDPP4 to positively infl uence the two earliest and most signifi cant links in the pathogenesis of type 2 diabetes.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>α- и β-клетки</kwd><kwd>эндокриноциты</kwd><kwd>сахарный диабет 2 типа</kwd><kwd>ингибиторы дипептидилпептидазы-4</kwd></kwd-group><kwd-group xml:lang="en"><kwd>α- and β-cells</kwd><kwd>endocrinocytes</kwd><kwd>type 2 diabetes mellitus</kwd><kwd>dipeptidyl peptidase-4 inhibitors</kwd></kwd-group><funding-group><funding-statement xml:lang="ru">Государственное задание 2021-03.
