Цель: обобщение нашего опыта диагностики и лечения острого мастоидита и его осложнений у детей.

Материалы и методы

Материалы и методы: в исследование включено 26 детей с острым мастоидитом. Осложнения наблюдались в 15,4% случаев.

Результаты

Результаты: больные были разделены на 3 группы: в 1 группу включено 13 детей, у которых выздоровление достигнуто применением парацентеза и антибиотикотерапии, во 2 группу – 11 детей, подвергнутых хирургическому вмешательству на сосцевидном отростке, и в 3 группу – 2 детей с латентным течением мастоидита, диагностированного после развития осложнений. Осложнения мастоидита обнаружены у 3 больных: субпериостальный абсцесс – у 1 больного, периферический парез лицевого нерва – у 2 больных.

Выводы

Выводы: осложнения острого мастоидита встречаются в тех случаях, когда aditus ad antrum блокирован отечной слизистой оболочкой или грануляционной тканью. Антибиотикотерапия, начатая при появлении симптомов острого среднего отита, не предотвращает развития осложнений.

Purpose

Purpose: To review our recent experience in the diagnosis and treatment of acute mastoiditis and its complications in children.

Materials and Methods

Materials and Methods: 26 children with acute mastoiditis were included in the study. Complications occurred in 15,4 % of episodes.

Results

Results: All patients were split into 3 groups. Group 1 included 13 children whose recovery was due to the application of paracentesis and antibiotic treatment; group 2 included 11 children who underwent mastoid surgical intervention; group 3 included 2 children with delitescent mastoid disease diagnosed after complications development. Complications were found in 3 patients: subperiosteal abscess – in 1 patient, peripheral facial palsy – in 2 patients with masked mastoiditis.

Summary

Summary: Complications of acute mastoiditis occur in cases where the aditus ad antrum is obstructed with edematous mucosa or inflammatory granulation tissue. Antibiotic treatment at the onset of symptoms of the acute otitis media does not prevent complications.
